Dear Parent/Guardian:
Welcome to Primrose School! If your child will be five years of age on or before December 31st, 2021 they will be eligible to start kindergarten in September 2021. Registration will take place online throughout the month of February. The registration window will open on February 1, 2021, and remain open through Friday, February 26, 2021.Before you begin the online registratio, please gather the following documents and scan them to your computer so you can upload them during the registration process:
- Proof of Student's Age
- Current Physical (not required for initial registration, but prefered)
- Immunization Record
- Proof of Residency
- Special Ed - IEP

Important Information from the Health Office

A physical exam and immunization record is required for all children entering school for the first time. This
physical must be done any time after DATE NEEDED.
Immunizations
All children are required by NY State to have ALL the following immunizations before entering school:• DTaP (or DPT) - 5 doses
• Polio (OPV or IPV) - 4 doses
• MMR - 2 doses
• Hepatitis B -3 doses on proper schedule
• Varicella/Varivax (chicken pox) - 2 doses OR doctor's note that the child has had the chicken pox disease.Reporting an absence
Please call our absence line (248-8926) to let us know that your child will be absent. This line is available 24
hours a day / 7 days a week! If your child is diagnosed with a contagious illness (i.e. strep, pink-eye, etc.), please
call the Health Office as soon as possible to let us know. This includes weekends and vacation weeks.Medications
Children are N.QI allowed to bring ANY medications to school. This includes common over-the-counter
medications such as Lactaid and Tylenol. If you would like our nurses to give your child ANY medication, please
fill out the appropriate form, which includes permission from your child's doctor. You must then bring the
medication to the Health Office.
• Cough drops and throat lozenges are not allowed due to the choking hazard
• Lip balm (such as Chapstick) is allowed and encouraged!Physical Education
Children need a note from their doctor to be excused from gym. We may excuse him/her for ONE day only
( or do "gym as tolerated") with a note from home. For any injury requiring a doctor visit, (such as stitches,
splints, etc.), please get a note from the doctor regarding gym and recess.
Recess
Children go outside every day, if the temperature is at least 15 degrees. Please dress your child appropriately.
No flip-flop shoes, please.
